Community Navigator / Case Manager

Position Summary:
Provides case management to clients of Alternatives Pregnancy Center. Directs and oversees case management services provided to clients. Completes social work assessment with client to determine resource needs and develop care plan for outcome focused results. Monitors and oversees client record-keeping and statistics. Effectively promoting the organization through increased visibility among target audiences and community building to create client-focused outreach efforts. Part of a larger client services team.

• A Bachelor’s Degree in related field including Social Work or psychology.
Preferred:
• Master’s Degree in Social Work or Counseling
• Experience in case management with vulnerable populations and/or crisis counseling
• Ability to speak Spanish

Part-Time 24-28 hours per week $22-$24 / hour

Experience Needed:   • A Bachelor's Degree in related field including Social Work or psychology. Preferred: • Master's Degree in Social Work or Counseling • Experience in case management with vulnerable populations and/or crisis counseling • Ability to speak Spanish
